We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. INTRODUCTION: Delusional misidentification syndromes (DMS) are a group of psychopathological experiences occurring in psychosis, involving the misidentification of a person or place. DMS are often accompanied by hostility towards the object of delusional misidentification. This is of a particular concern in perinatal mental illness due to the potential disruption of the mother-infant bond, and risk of neglect, violence, or infanticide towards a misidentified child. This review aimed to collate all published cases of DMS in postpartum psychosis to further understand how (...) these syndromes present in perinatal mental illness. METHODS: In August 2021, an online database search was conducted using PubMed, MEDLINE, PsycINFO, CINAHL, and Embase to identify all publications reporting DMS in the perinatal period. RESULTS: Nine papers were included in the review involving 8 case reports of Capgras syndrome and one case series involving 4 cases of Fregoli syndrome. Three cases identified organic pathology, which may have contributed to the presentation. The most common subject of misidentification was the patient’s husband (n = 7), followed by their baby (n = 6), hospital staff (n = 4), other family members (n = 3), and self (n = 1). Five cases remark on the impact of perinatal illness on the maternal-infant bond, of which four result in the mother being unwilling to care for the infant as the result of their delusional beliefs. CONCLUSION: This is the first systematic review of the literature in this field. Although small in number, these cases reveal several important learning points including that DMS can occur with or without underlying organic disease. Active exploration of the nature of delusions in postpartum psychosis is required to mitigate the risk of harm to the infant and mother-infant bond. It may also uncover that these syndromes are more common in postpartum psychosis than previously realized. (shrink)

L’essor des réseaux sociaux a permis aux influenceurs de façonner le discours sur la santé publique et de promouvoir des produits et des interventions de santé non éprouvés. Nous examinons ici les défis pratiques et réglementaires ainsi que les opportunités liés au marketing de la santé mené par les influenceurs aux États-Unis et au Canada. Malgré les interdictions légales existantes concernant la publicité trompeuse directement liée aux influenceurs, telles que définies par les directives réglementaires officielles, leur application reste insuffisante. Les (...) cadres réglementaires actuels autorisent des allégations de santé vagues. De plus, la surveillance diffère entre les influenceurs professionnels de santé agréés et les influenceurs profanes. Les plateformes de réseaux sociaux sont complices ou indifférentes à l’amplification de la désinformation, car celle-ci stimule l’engagement et sert leurs motivations lucratives. Pour lutter contre le marketing de santé trompeur des influenceurs, des stratégies à la fois « dures » et « douces » sont envisagées, notamment la coordination des politiques internationales et des solutions algorithmiques qui réduisent la visibilité des contenus trompeurs. Un défi majeur reste de savoir comment susciter la participation volontaire de puissantes multinationales disposant d’une influence politique et de lobbying. Nous soulignons l’utilité potentielle de mécanismes d’application automatisés soigneusement conçus, notamment des systèmes de surveillance des contenus basés sur l’IA et des procédures automatisées de notification et de retrait, tout en plaidant pour une approche équilibrée qui respecte la liberté d’expression. Il est nécessaire de mettre en place une réglementation et des mécanismes d’application actualisés, évolutifs et équitables afin de protéger les consommateurs, de favoriser une communication en matière de santé fondée sur des données probantes et de rétablir la responsabilité dans le marketing de la santé numérique. The rise of social media has enabled influencers to shape public health discourse and promote unproven health products and interventions. Here we examine practical and regulatory challenges and opportunities relating to influencer-driven health marketing in the United States and Canada. Despite existing legal prohibitions on misleading advertising linked directly to influencers by official regulatory guidance, enforcement remains inadequate. Current regulatory frameworks permit vague health claims. Moreover, oversight differs between licensed health professional influencers and layperson influencers. Social media platforms are complicit (...) in and/or indifferent to the amplification of misinformation, as it drives engagement and furthers their profit motives. To address misleading influencer health marketing, both “hard” and “soft” strategies are considered, including international policy coordination and algorithmic solutions that reduce the visibility of misleading content. One key challenge remains how to engender the willing participation of powerful multinational corporations with political and lobbying influence. We raise the potential usefulness of carefully crafted automated enforcement mechanisms, including AI-driven content monitoring systems and automated notice-and-takedown procedures, while simultaneously advocating for a balanced approach that respects freedom of expression. Updated, scalable, and equitable regulations and enforcement mechanisms are needed to protect consumers, foster evidence-based health communication, and restore accountability in digital health marketing. An alleged moral right to informational privacy assumes that we should have control over information about ourselves. What is the philosophical justification for this control? I think that one prevalent answer to this question—an answer that has to do with the justification of negative rights generally—will not do.

Mr Olding's recent attack on my exposition of the argument from design gives me an opportunity to defend the central theses of my original article. My article pointed out that there were arguments from design of two types—those which take as their premisses regularities of copresence and those which take as their premisses regularities of succession. I sought to defend an argument of the second type. One merit of such an argument is that there is no doubt about the truth (...) of its premisses. Almost all objects in the world behave in a highly regular way describable by scientific laws. Further, any scientific explanation of such a regularity must invoke some more general regularity. The most general regularities of all are, as such, scientifically inexplicable. The question arises whether there is a possible explanation of another kind which can be provided for them, and whether their occurrence gives any or much support to that explanation. I urged that we do explain some phenomena by explanation of an entirely different kind from the scientific. We explain states of affairs by the action of agents who bring them about intentionally of their own choice. Regularities of succession, as well as other phenomena may be explained in this way. Explanation of this kind I will term intentional explanation. Intentional explanation of some phenomenon E consists in adducing an agent A who brought E about of his own choice and a further end G which, he believed, would be forwarded by the production of E. Minnesota Archive Editions uses digital technology to make long-unavailable books once again accessible, and are published unaltered from the original University of Minnesota Press editions. At issue in the clash between utilitarianism and the theory of rights is a fundamental question about the theoretical underpinnings of moral and political philosophy. Is this structure to be utility-based—grounded in the general welfare—or is it to be based on individual moral and political rights, as critics (...) of utilitarianism increasingly insist? The argument centers, in part, upon the fact that utilitarianism, with its emphasis upon outcomes and total utility in the world, seems to employ a value theory that offers no protection to persons and their vital interests. This book aims to provide an introduction to Aristotle's Politics, highlighting the major themes and arguments offered in the scholar's work. It begins with a discussion on what Aristotle perceives as human good, which he had described as the ethical purpose of political science, and how he views the political community, or the polis, as a community of persons formed with a view to some good purpose and a supreme entity in the sense that it is not just one aspect (...) of city-state society but is the whole of that society. The book also discusses Aristotle's arguments against the abolition of the household for the ruling class and the communism of families and property, which Plato advocates, and highlights certain inconsistencies and vulnerabilities in his arguments supporting natural slavery and on how the husband should rule over his wife and children. The book presents Aristotle's account of the relative merits of six different types of constitution and his views on the best possible constitution that will help in the realization of the polis' true purpose of achieving the good life. In this article we show that it is possible to completely classify the degrees of r.e. bases of r.e. vector spaces in terms of weak truth table degrees. The ideas extend to classify the degrees of complements and splittings. Several ramifications of the classification are discussed, together with an analysis of the structure of the degrees of pairs of r.e. summands of r.e. spaces.

This early work by Robin G. Collingwood was originally published in 1924 and we are now republishing it with a brand new introductory biography. 'Speculum Mentis' is an academic work on the subject of philosophy. Robin George Collingwood was born on 22nd February 1889, in Cartmel, England. He was the son of author, artist, and academic, W. G. Collingwood. He was greatly influenced by the Italian Idealists Croce, Gentile, and Guido de Ruggiero. Another important influence was his father, a professor (...) of fine art and a student of Ruskin. Collingwood published this article the same year that he published his first book on Aesthetics: "Outlines of a Philosophy of Art". The article can be divided in two main sections. In the first one Collingwood defends the existence of a Philosophy of Art in Plato's Republic, in close relation to the theory of reality expounded by Plato in the Book. From Collingwood's point of view, Plato understood art as "an appearance of an appearance", closely related to imagination, and as a (...) symbol of truth. An Essay on Metaphysics (1940) is one of the finest works of the great Oxford philosopher R. G. Collingwood (1889-1943): in it he considers the nature of philosophy, especially of metaphysics, and puts forward his original and influential theories of absolute presuppositions, causation, and the logic of question and answer. When travellers are overcome by cold, it is said, they lie down quite happily and die. They put up no fight for life. If they struggled, they would keep warm; but they no longer want to struggle. The cold in themselves takes away the will to fight against the cold around them.

A number of ordinarily separate disciplines--e.g., physics, physiology, psychology--are here brought together in an effort to reconstitute optics as the complete science of human vision, thus replacing classical optics which dealt with vision only under perfect conditions. Informed choice is increasingly recognised as important in supporting patient autonomy and ensuring that people are neither deceived nor coerced. In cancer screening the emphasis has shifted away from just promoting the benefits of screening to providing comprehensive information to enable people to make an informed choice. Cancer screening programmes in the UK now have policies in place which state that it is their responsibility to ensure that individuals are making an individual informed choice. There is a need to evaluate (...) whether such policies mean that those people invited for screening are making informed choices, and how comprehensive information affects other variables such as uptake, cost effectiveness, and satisfaction. At the present time, there is no validated measure of informed choice in cancer screening. Such a measure could be used to evaluate the effectiveness of interventions to increase informed choice and levels of informed choice in a population invited for screening. It could encourage health professionals to be accountable. Factors important when measuring informed choice in cancer screening include an individual’s understanding of the limitations of screening, the ability to make an autonomous choice, and the difference between choice and behaviour.
